Housing Application
Below you will find instructions and links to the documents that must be completed and returned in oder to apply for housing at The Baptist Manor. Please ensure that each person applying completes a duplicate form.
- The Application (PDF, 120K)- this is to be completed by each applicant. Please note that the person who lists their name first is considered "the head of household" The second name on the application is the co-applicant.
- Release Form for Criminal and Credit Check (PDF, 84K) - each applicant must complete and sign this form.
- Tenant Declaration Form (PDF, 100K) - each applicant must complete the three-page packet. If you are a US citizen, complete the first and third page
If you are a Permanent Resident Alien, complete the second and third page. The third page is titled “Family Summary”. Here you list the persons who will be living in the apartment. Please do not give a list of family members.
- Document Package for Applicant’s Consent to the Release of Information (PDF, 59K) - each applicant must sign and date the second page. Each applicant must also submit the back page signed.
- Release Form for Landlord Verification (PDF, 64K) - each applicant must complete and sign this form.
